    Brow Lamination and Hybrid Tint After Care
    - Avoid water, steam and chlorine on the brows for 24/48 hours
    - Avoid shampoo, makeup, conditioners, makeup remover/cleanser, creams, serums for 24/48 hours
    - Avoid touching or rubbing your eyebrows for 24/48 hours

    I have read, understood and agree that I have provided correct & truthful information. I acknowledge that & understand that there are potential risks of harmful or negative side effects associated with having the procedure. I have read and understand the post-treatment home care instructions & will follow recommendations made by Lartista. In the event that my circumstances have changed, additional information, questions or concerns regarding my treatment or suggested home product / post-treatment care will be disclosed with Lartista immediately. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I affirm that I have had sufficient opportunity to provide information and discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold Lartista responsible for any of my conditions that were present or otherwise, and take responsibility for conditions not disclosed that may be affected by the treatment. I understand there are risks associated with having this procedure. I understand that there has a NO REFUND policy on all services, procedures, appointments and products. I agree I will not receive a refund. I understand photos and/or recordings of specialist's notes &/or promotional content may be taken before, during or after my appointment. I agree to notify my specialist should I not consent to be used in promotional content.

    I understand and agree that by submitting this online form I am entering into a legal & binding agreement and hereby release any and all persons representing L'artista from all claims, demands, damages, actions and cause of action arising out of the performance of the service.
